Access to Health Services under the Universal Coverage Policy among Elderly in Khon Kaen Municipal Area(การเข้าถึงบริการสุขภาพภายใต้โครงการหลักประกันสุขภาพถ้วนหน้า ของผู้สูงอายุในเขตเทศบาลนครขอนแก่น)

Abstract

การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อ อธิบายการใช้บริการสุขภาพภายใต้โครงการหลักประกันสุขภาพถ้วนหน้า ของผู้สูงอายุในเขตเทศบาลนครขอนแก่น กลุ่มตัวอย่าง คือ ผู้สูงอายุที่มีสิทธิภายใต้โครงการหลักประกันสุขภาพถ้วนหน้า จำนวน 225 คน ผลศึกษาพบว่า ผู้สูงอายุ ร้อยละ 49.8 เจ็บป่วยเล็กน้อย ใน 1 เดือนที่ผ่านมา เจ็บป่วยหนัก ร้อยละ 24.4 และมีปัญหาเกี่ยวกับสุขภาพช่องปากและฟัน ร้อยละ 47.1 ในระยะ 1 ปีที่ผ่านมา ผู้สูงอายุไปใช้บริการสุขภาพสถานบริการที่ระบุในบัตรทองเมื่อเจ็บป่วยเล็กน้อย (ร้อยละ 60.9) เจ็บป่วยหนัก (ร้อยละ 83.7) และเมื่อมีปัญหาเกี่ยวกับสุขภาพช่องปากและฟัน (ร้อยละ 55.7) ส่วนการใช้บริการด้านส่งเสริมสุขภาพ ผู้สูงอายุ ร้อยละ 24.9 เคยไปรับบริการขอคำแนะนำด้านสุขภาพจากสถานบริการ และผู้สูงอายุ ร้อยละ 44.0 เคยได้รับการออกเยี่ยมบ้านจากเจ้าหน้าที่สาธารณสุข สำหรับการใช้บริการด้านการป้องกันโรค ผู้สูงอายุมากกว่าครึ่งหนึ่ง (ร้อยละ 56.5) ได้รับการตรวจสุขภาพประจำปี และการตรวจคัดกรองโรคเรื้อรัง ได้แก่ โรคความดันโลหิตสูง โรคเบาหวาน โรคหัวใจ ร้อยละ 64.5, 59.8 และ 45.8 ตามลำดับ ส่วนการใช้บริการด้านการฟื้นฟูสภาพ พบว่าผู้สูงอายุ ร้อยละ 55.1 ใช้อุปกรณ์หรือกายอุปกรณ์ช่วยเหลือในกิจกรรมต่างๆ ประจำวัน ซึ่งอุปกรณ์ที่ใช้ส่วนใหญ่ คือ แว่นตา รองลงมาเป็นฟันปลอม

This descriptive cross-sectional study aimed to explain the health service utilization under the universal coverage health insurance scheme (UC) of the elderly in Khon Kaen municipality. The 225 elderly under the UC were systematic randomly selected to response to questionnaires interviewed. The results revealed that 49.8 % of the elders had mild sickness in the past month and 24.4 % suffered with severe sickness in the past year. Furthermore, almost half of them (47.1 % percent) had gum and teeth problems. When had mild problems 60.9 % of them seek service from health service registered in their UC. Most of these elders who suffered with severe illness (83.7 %) got serviced from UC health facilities, whereas only 55.7 percent seek services from UC health facilities when had gum and teeth problems. Only 24.9 % of the elders received health promotion services, 44 % got home health care during the last 12 months. More than half of them (56.5 %) got physical check up during the past year, Most of them (64.5 %) got blood pressure checked, 59.8 % had diabetes mellitus checked and 48.5 % of them were undergone electro cardiograph (EKG). In term of rehabilitation, 55.1 % of the elders used assisting aid, mostly eye glasses and artificial teeth.
