Abstract

This action research was to develop health service delivery for people with diabetes at Don Chang Subdistrict Health Promoting Hospital. The results show as follows: 1) participants with diabetes could not control their diets, do not regularly  exercise and hold stress; hence   they could not manage their blood glucose levels. 2) Caregivers and village health workers still lack knowledge and skills in taking care of patients.  3) The practice guidelines of diabetic clinics are not clear, so the service may be delayed; and the data are not recorded systematically and completely. The problems may be solved as follows: 1) The clinics increased more days of services; and provide clearer practice guidelines and grouped the patients. 2) The data collection form has not been updated to cover complete data of patients. The development resulted in more satisfaction of patients for the health care service they got. The solutions corresponded to the problems and needs. In addition, the patients could easily and quickly reach the service and did not need to wait for a long time. Apart from that, the caregivers got clearer practice guidelines. The data was collected systematically, completely and up to date.
