The purposes of this research were 1) to design a horror game according to the level of fear and 2) to develop a horror game using the Unreal Engine 4. The 10 levels divided according to the level of fear, including the Level 1 Calm is a safe exploration of the map, Level 2-4 Anxiety will start with an unknown sound origin soundtrack and appearance of blood or objects stained with blood Including objects running in front or falling from a height, Level 5-6 Stress is hearing footsteps following or the appearance of objects quickly, Level 7-8 Fear of changing the surrounding environment, such as light, dark sky or weather Including the presence of more violent objects, Level 9 Fearful Chases and Level 10 Panic is either caught or hit until death or the full screen image appears quickly. The design takes into account factors that affect player enjoyment in 8 topics: concentration, challenge, player skill, control, goal, feedback, imersion and anxiety. The tools used in the research were Unreal Engine 4, SketchUp, Adobe Premiere Pro and Blender etc.

The results showed that 1) Level design of horror games to cope with situational anxiety was divided into 10 levels according to Level of Fear which is the level design of horror games such as complete peace without fear, Fear caused by enemies chasing, etc. 2) developing horror games to deal with situational anxiety, developing horror games with Unreal Engine 4 used to develop games with Blueprint, SketchUp used to create maps, Adobe Premiere Pro is used to edit audio and Blender is used to create character models. Including using the concept of anxiety based on various situations that the player encounters in the game to create an atmosphere and feeling associated with the game.
